Wellness Benefit
This benefit pays $75 per
calendar year per covered Voluntary Critical Illness Coverage
member if a covered
health screening test is Critical illness coverage, available through Unum, is designed to help you offset the

tests, chest x-rays, stress inancial effects of a catastrophic illness with a lump sum beneit if you or a loved
tests, mammograms, and one are diagnosed with a covered critical illness. The critical illness beneit is based
colonoscopies. A full list of
covered tests will be provided on the amount of coverage in effect on the date of diagnosis of a critical illness or
in your certificate. the date treatment is received according to the terms and provisions of the policy.
During beneits enrollment, you can elect this coverage without medical questions.
X Employee: up to $20,000
X Spouse: up to $10,000
X Child(ren) are covered at 25% of the employee-level amount, for no additional
cost
X Coverage is portable which means you can take this plan with you if you no
longer work for the company
X Diagnosis must occur at least 30 days after the coverage effective date to be
eligible for beneits
X A 12-month pre-existing condition exclusion applies if you did not previously
have critical illness coverage; see plan document for details
