Electric Power Systems has offered the same STD and LTD beneit levels
for several years. In response to inlation, cost of living, and employee
compensation changes, EPS feels it is wise to offer employees increased
beneits levels for both STD and LTD.

Voluntary Short Term Disability

Insurance

Voluntary short term disability (STD) is a beneit which protects your income by

providing you with a percentage of your income while you are unable to work due
to a non work related accident or sickness. The beneit is equal to 60 percent of your
weekly base pay to up $1,500 and may continue for up to 11 weeks. The monthly rate
is $0.19 per $10 of weekly beneit. This beneit is offered through Unum.

Note evidence of insurability (EOI) or proof of good health is required if you do
not elect coverage during the current beneits enrollment period.

Long Term Disability Insurance

Long term disability (LTD) is provided by EPS at no cost to the employee. After a

90 day waiting period, this beneit is equal to 60 percent of your monthly base pay.
In the event of an illness or accident which requires a lengthy recovery, you may be
eligible for company-provided long term disability coverage. This beneit is offered
through Unum.
