Benefits Eligibility
Eligible Employees
You may enroll in the beneits program if you are a regular full-time
employee who is actively working a minimum of 30 hours per week.
New Hire Benefits Effective Date
You are eligible to enroll in beneits from your date of hire through your
30th day of employment. You can enroll in beneits by logging into the
ADP/WFN portal. If you do not enroll by your 30th day of employment,
you may not enroll until the next annual enrollment period unless you
have a qualiied life status change as noted on page 3.
Eligible Dependents
You may enroll your legal spouse and dependent children to age 26
regardless of student, marital status or inancial dependency. If your legal
spouse is employed outside the home and is eligible for coverage with
their employer, they should enroll in their employer’s plan. Dependent
children include natural, legally adopted, step and/or foster children, as
well as children for whom you are the legal guardian.
SSN, Birth, and Marriage Certificate Required
You must provide a valid Social Security Number for yourself and each
enrolled dependent. Employers are required to provide names and Social
Security Numbers to the federal government for each individual enrolled
in medical coverage.
Birth certiicates/orders of adoption are required for all enrolled
dependents
If you are enrolling a spouse, a marriage certiicate is required
