The Wellness Benefit is a rider that is included with your Accident Insurance
coverage. It provides an annual benefit payment if you complete a health
screening test on or after your coverage effective date. You will receive:
$50 for yourself
$50 for your spouse
$25 per child, up to a maximum of $100 per year for all children
