Medical Plan
Premier Performance Products partners with BCBS of Idaho to establish a network
of physicians and facilities for our employees to access. If you visit doctors or
hospitals in the network, you will pay less. For example, an ofice visit to your
primary care physician is $30 (your copay) if the doctor is in the network. Of course,
you may always use any medical provider you like, but keep in mind in-network
providers will cost you a lot less.
If you choose an in-network provider, you pay a copayment for doctor’s ofice visits
and 20% of charges after you meet the deductible.
For most other services, if you choose an out-of-network provider, it is your
responsibility to pay the out-of-network provider and to ile your claims to the
insurance carrier for reimbursement. In addition, you may be responsible for any
charges which exceed the customary and reasonable amounts.
Preventive care is covered at 100% when care is received through an in-network
physician or facility.
