Children 6 months to 26 years (if student)—Flat
$10,000
The maximum the employee can purchase is
$150,000 without having to complete a personal
health application and the maximum the employee
can purchase for spouse coverage is $30,000 without
having to complete a personal health application.
An employee can elect voluntary child life without
electing voluntary spouse life. The voluntary spouse or
voluntary child amounts elected cannot exceed 50%
of the voluntary employee beneit amount. The cost
of this coverage is determined by your age. Please see
