committed to offering competitive beneit programs that help us attract and retain our team members.
In 2015, we will continue to offer our traditional PPO Plan and are pleased to let you know that the PPO rates
will be the same as they were in 2014. Last year we introduced a new medical plan called a Health Savings
Account (HSA) Plan. This plan is more commonly known as a Consumer Driven Health Plan (CDHP). The
premiums are less than our traditional PPO Plan and the plan includes a personal healthcare bank account to
help with future medical expenses. In 2015, the HSA rates will be lower than they were in 2014, and we will
once again contribute $300 (employee only) or $600 (family coverage) to your personal healthcare bank account
throughout the year to help you pay for some of your medical expenses. Our HSA Plan is designed to incentivize
team members to sit in the driver’s seat and take control of their healthcare spending.
As you read about the beneits we are offering in 2015, I encourage you to research your options before making
a decision. For questions about our beneits programs, you can call the Beneits Department at 1-800-436-9556.
