BASIC LIFE AND AD&D
Designating Your
Beneficiary
If you pass away while covered Basic group term life and accidental death and dismemberment (AD&D)
by this plan, the beneit is paid to insurance is provided to you at no cost through Lincoln Financial. This

6 months to 26 years in $2,500 units to $10,000
On the date you attain age 65, the beneit reduces to 65% of your
original beneit; when you reach age 70, your beneit reduces to
50%; when you reach age 75, a inal reduction occurs to 25% of
your original beneit

Beneits enrollment is not ofered this year. However, coverage may be
requested by completing an evidence of insurability (EOI) form.
