Complete a biometric screening at a 07/01/2021 – 11/30/2021
LabCorp clinic near you.
All Team Members: Complete all 3
steps by 11/30 to be entered into a
drawing for a GRAND PRIZE in
Step 3: Earn 500 Lifestyle Rewards Points
Participate in healthy activities from the December! Lifestyle Rewards:
Lifestyle Rewards activity list to earn Health Plan Team Members: 07/01/2021 – 11/30/2021

• Team members on the health plan who complete the biometric screening, health survey, and achieve 500
Lifestyle Rewards points by 11/30 will be eligible for the wellness premium rates in 2022. Medical premiums
will be discounted by $23 per pay period starting January 2022 – you can save up to $600 per year!
If you are unable to meet a health outcome for an incentive under the HealthCheck360 Program, you can work with HealthCheck360 for an
opportunity to earn the same incentive through a reasonable alternative process. To speak with a representative about what options are
