You’ll now have the option to complete your screening at a LabCorp clinic or
with your physician. Follow the instructions below for LabCorp.
REGISTER FOR YOUR LABCORP SCREENING
COMPLETE YOUR • Log into your myHealthCheck360 app.
SCREENING BY: • Select More, then select Schedule a Screening.
11/30/2021 o Select Offsite Lab then enter your address to find a LabCorp nearest to you.
The list of available LabCorp locations will show.
To choose one, click Select.
• Accept the Health Screening Consent and HIPAA Authorization forms by clicking the I
Agree box located at the bottom of each consent. In the signature section, enter your
first and last name and click Proceed to Confirmation.
• You will receive an email confirmation for your screening from
service@ehealthscreenings.com.
o This confirmation email includes the lab form and screening instructions. You
must take these with you to complete your screening.
• Once you reach the confirmation page/email, please register for an appointment
time at the clinic you chose.
o Go to LabCorp.com, type in your address/zip code, and select employee
wellness with body measurements.
o Once you register for an appointment at LabCorp, you will receive a confirmation
email from email@labcorp.com.
