Apricity Benefits Overview
VOLUNTARY CRITICAL ILLNESS INSURANCE
If serious illness strikes you or your family, the last thing you need to worry about is immediate
medical expenses. This plan helps you focus on recuperation instead of the distraction and stress
over out-of-pocket costs. With the critical illness plan, you receive cash beneits directly (unless
otherwise assigned)—giving you the lexibility to help pay bills related to treatment or to help with
everyday living expenses. Critical illness beneits through MetLife are payable for:
Covered Condition Initial Beneit Recurrence Beneit
Alzheimer’s Disease 100% None
Coronary Artery Disease 100% None
Full Cancer Beneit 100% 100%
Partial Cancer Beneit 25% 25%
Heart Attack 100% 100%
Kidney Failure 100% None
Major Organ Transplant 100% 100%
Stroke 100% 100%
Skin Cancer 5% None
Coverage is available for you, your legal spouse, and children from live birth to age 26. The plan
contains a 30-day waiting period, which means no beneits are payable for anyone who has been
diagnosed before your coverage has been in force for 30 days from the effective date. See the
plan summary for more details and limitations.
In addition, the plan includes a health screening beneit that pays each covered member $50
per year. You must be enrolled in the plan for at least 30 days and receive a health screening
before this beneit is payable. Remember, an annual wellness visit with your doctor is covered at
no cost to you under our medical plans (in-network). That’s an extra $50 in your pocket for taking
advantage of this free visit!
Coverage Levels
Employee: $5,000 increments up to $50,000.
Spouse: Equal to 100% of employee amount.
Child: Each dependent child is covered at 50% of employee amount.
