Health Incentive Program


The Envision AESC Health Incentive Program is an optional beneit
where employees in the Savings (HSA) plan and Reimbursement (HRA)
plan can earn funding for healthcare expenses.

How to Earn Incentives


To earn the annual health incentive funding to your HSA (if enrolled in
the Savings plan) or HRA (if enrolled in the Reimbursement plan), you
and your covered spouse must complete the personal health assessment
(PHA) through BlueAccess.


The PHA is a conidential survey that allows you to evaluate your overall
health and wellbeing and suggests goals to help improve areas of concern
or interest.


To complete the PHA, log into BlueAccess and go to My Health &
Wellness. You will receive a pro-rated share of the annual funding shown
below based on the number of months left in the year after you and your
covered spouse (if applicable) complete your PHA. You can expect the
funds to be deposited by the end of the month following the month that
you complete your PHA.
