If you complete the Tobacco Cessation Program, you
will no longer be required to pay the surcharge. To
have the surcharge removed, contact ASKHR and
notify a representative about your change in tobacco
use status. You will also need to provide a certiicate
of completion from the Tobacco Cessation Program.
If you start using tobacco during the year, you are
required to contact the Brady Beneits Center and
begin paying the surcharge.

why Brady ofers Quit For Life. This program is
available at no cost to employees and dependents
age 18 and older, regardless of which plan you
enroll in.
