Accident Coverage
Plan covers accidents that occur outside of work.
File a claim, receive reimbursement!
Sample of Benefits:
Up to $5,000 for Fractures (broken bone)
Up to $4,000 for Dislocations (separated joint)
$1,000 for Hospital Admission + $200 for Hospital Confinement per day
$150 for Emergency Room, Urgent Care or Primary Care Physician
$200 for Diagnostic Exams (Angioram, CT Scan, CTA Scan, MRI, MRA or EEG)
Up to $150 annual Wellness Benefit per insured for certain preventive
screenings (up to $450 per family)
