Universal Life Coverage
Basic Group Variable Universal Life (GVUL)
The Company provides you with GVUL coverage equal to one times
your base annual salary rounded to the next higher $1,000 (if not already
a multiple thereof) to a maximum of $400,000. You have the opportunity
to contribute extra premiums for investment into your certiicate. These
extra premiums have the potential to grow tax-deferred and build cash
value. You may invest these extra premiums in several different portfolios.

Accidental Death & Dismemberment (AD&D)
Insurance
The Company provides you with one times your annual salary to a
maximum of $400,000 in AD&D insurance. This coverage is at no cost to
you and is offered through Mutual of Omaha.
Supplemental Group Variable Universal Life
(GVUL)
Employees have the opportunity to elect additional GVUL coverage for
themselves, their spouses, and dependent children.
If you elect coverage or increase coverage during open enrollment, you
will be required to complete an Evidence of Insurability form. Guarantee
Issue applies only to persons in their newly eligible period.
