Supplemental GVUL Insurance Coverage
Employee
Multiples of annual salary;
Maximum of the lesser of ive times annual salary or $2,000,000 (less
basic coverage amount)
Guarantee issue: two times annual salary
Spouse (Term Life)
Increments of $5,000 from $10,000 to a maximum of the lesser of
100% of the employee amount or $250,000
Guarantee issue: $50,000
Spouse must be age 70 or under at issue; spouse coverage terminates
on the policy anniversary date closes to the earlier of the employee or
the spouse’s 75th birth date
Child(ren) (Term Life)
Dependent children coverage is available in the following amount:
$10,000
Guarantee issue: $10,000
Children must be aged 15 days through 18 at issue (though 21 if
full-time student); coverage terminates on the policy anniversary date
closest to the child’s 25th birth date
Note: Your beneiciary information must be acknowledged and accepted by MetLife. Contact

Optional AD&D Insurance
You have the opportunity to purchase additional AD&D insurance. The
coverage is through Mutual of Omaha and provides beneits if you lose
your life, become paralyzed or are dismembered due to an accident.
You may elect an amount from $10,000 up to $500,000. Coverage
can be purchased for yourself only or for your family. The monthly
cost is $0.03/$1,000 of coverage if you elect employee coverage and
$0.04/$1,000 of coverage if you elect family coverage.
