Guarantee Issue Offer!

Employees who previously declined coverage will be able to purchase
1 times salary up to $300,000 without completing health questions
during this open enrollment period. Amounts over the 1 times salary
amount will be subject to health questions .

Employees who are currently enrolled in this plan will be able to
increase their election by 1 times salary up to the guarantee issue
maximum which is 2 times salary up to $300,000 during this open
enrollment period only.
