Voluntary AD&D 2017 Beneits Enrollment
Voluntary accidental death and dismemberment insurance provides a beneit to you
or your loved ones should you be injured or die as the result of an accident. You will
see these beneits as a single deduction on your paycheck.
Voluntary AD&D
Employee $0.015 per $1,000 of coverage
Spouse $0.025 per $1,000 of coverage
Dependent Child $0.025 per $1,000 of coverage
To determine the monthly premium for voluntary life coverage, follow these simple
steps.
1. Determine the amount of insurance you want to purchase
2. Using the table, determine the rate based on your age
3. Use the following formula to calculate your monthly premium.
(amount of coverage you wish to purchase/$1,000) x (your age banded rate)
