family member experienced a covered accident, illness, or hospitalization. Symetra’s Accident and Critical Illness
coverage can supplement your health plan and provide you and your family with the additional inancial protection
you may need. These plans pay beneits directly to you and you decide how to use the beneit.
Accident Coverage Critical Illness Coverage

Physical Therapy (six sessions) $180 in WorkDay when going though enrollment.

Total Dollars Payable to Employee $1,470 Wellness Benefit

wellness beneit that pays $50 per calendar year per
insured individual if a covered health screening test is
performed, including blood tests, chest x-rays, stress tests,
mammograms, and colonoscopies. A full list of covered tests
will be provided in your certiicate.
