Medical Coverage with Anthem
Our company must continue responding to the national trends of rising
healthcare costs, including some costs driven by healthcare reform. Even
with the changes we have made over the past several years, healthcare
costs continue to be a challenge. To ensure high-quality healthcare
remains affordable and sustainable for the company and our employees,
we are introducing important changes to our program this year.
What to Expect in 2016
CDHP with an HSA is not changing plan design, employee
contribution, or HSA company paid contribution
Classic PPO is increasing deductibles, out-of-pocket maximums, and
coinsurance for in-network and out-of-network coverage; employee
contributions are also increasing
Premier PPO is increasing deductibles, out-of-pocket maximums,
and employee contributions
Your Proof of Healthcare Coverage
The Affordable Care Act, in most cases, requires
individuals to purchase healthcare coverage. In
January, you will receive Form 1095-C confirmation
of your medical insurance election for 2015. This
confirmation will serve as your proof of healthcare
coverage and compliance with the Individual
Mandate.
