Eligibility
Eligibility
All full-time employees are eligible for beneits described in this Guide.
Regular part-time employees are eligible for beneits on a pro-rated basis.
Dependents are eligible for certain beneits according to the following
eligibility guidelines:
Type of Dependent Eligibility Requirements
Spouse Must be legally married to the employee
Biological Child Under age 26
Adopted/Foster Child Under age 26
Step-Child Under age 26
Child Covered Under a Order must be a Qualiied Medical Child
Qualiied Medical Child Support Support Order (QMCSO)
Order (QMCSO)
Disabled Child Age 26 or Older z Must be incapable of self-sustaining
employment because of physical, mental,
or other developmental disability, mental
illness, or mental health disorder
z Dependent on the employee for
a majority of inancial support and
maintenance and lives with employee for
more than half the year
z Was covered under the Plan before
turning age 26
