Critical Illness Coverage
Critical illnesses can be a major detriment to your income with increased
health expenses and the cost of daily living. This plan pays a lump sum,
directly to you, for any of the below conditions.
Elect now, without any medical questions*:
Employee: $10,000 or $20,000
Spouse: $5,000 or $10,000
Child(ren): $5,000 (one lat rate for all children)
* If you choose to elect coverage at a later date, evidence of insurability will be required .
