If you qualify to contribute to an HSA and are enrolled in Centennial’s
QHDHP, Centennial will deposit pre-tax contributions you elect from
your paycheck into your HSA. The Company may also contribute towards
the Employee HSA account based on employee grade level and by
coverage type (see chart below).

Oficers $0 $0
Company contributions will be deposited two times per plan year and
will be prorated based on eligibility during the plan year. Please note
Centennial’s contributions count toward the annual limits listed on this
page.
Executive Management reserves the right to eliminate or modify these
contribution at any time to ensure that Centennial meets its management
objectives.
Qualiied health expenses which may be reimbursed from an HSA on a
tax-free basis are listed in IRS publication 502 and include out-of-pocket
medical, dental, and vision expenses for you and your dependents.
