Find a UHC In-Network Provider
It is highly recommended you verify all associated providers are paricipaing in UHC’s network. This not only includes
your doctor, but also anesthesiologists, lab faciliies, etc. This will help you avoid incurring any unexpected out-of-
network charges and ensure the most cost efecive use of your health plan.
Follow these steps to ensure your providers are in-network:
1. Visit www.myuhc.com
2. Click on “Find Physician, Laboratory or Facility” under Links and Tools
3. You can narrow your search results by also selecing your network, “Choice Plus Plan”
4. Enter the provider or facility you want to verify in the “New Search” bar
UnitedHealth Premium® Designation Program
UnitedHealthcare developed the UnitedHealth Premium designaion program, which recognizes physicians that meet
guidelines for providing quality and cost eicient care.
The program uses naional industry standards to evaluate for quality and local market benchmarks for cost eiciency
across 27 Specialiies, including family pracice, internal medicine, pediatrics, cardiology and orthopaedics.
When searching for a Network provider, look for the Tier 1
indicator. ID
Cards
You may print a temporary
ID card shortly ater elecing coverage by
logging onto www.myuhc.com. Once you register click
on account seing and then select print an ID card. You
will also be able to look up a doctor, access your claim
informaion, look up a cost of prescripion and more on
the UHC website. Please note, you will receive the hard
copy of your medical card within 10-15 business days
ater you elect coverage.
