Eligibility Active full-time employees will be covered on the irst of the
month following your date of hire
AD&D Insurance
Basic Employees receive 1x annual salary up to a maximum of
$500,000
Supplemental If you elect voluntary life insurance you will receive an AD&D
beneit with a principal sum equal to the voluntary life amount

Guaranteed Issue Amounts
Employee Lesser of $250,000 or 5x basic annual earnings
Spouse $50,000
