X We have two health insurance carriers to choose from in our
medical program; our account based plans are offered through
UnitedHealthcare and the medical plans are based on United’s
Choice Plus Network; our HMO option is offered through Kaiser
Permanente
X The UHC “account-based” plans help you better manage your
healthcare costs and needs
X The UHC plans recognize and reward healthy lifestyles; not only is in-
network preventive care covered at 100%, you’ll also be able to earn
money provided by the company toward your beneits by completing
steps to improve your health
Changes for 2015
X We have partnered with an outside wellness vendor to help us better
reward and empower your positive lifestyle changes; you’ll receive
more information about our wellness vendor, Vitality, in early 2015
X When you participate in the new wellness program in 2015, you are
eligible to receive a higher employer paid contribution toward your
HRA or HSA account; if you have individual coverage you can earn
up to $700 and if you have family coverage you and your spouse can
earn up to $1,400 *
X Important: participating in the wellness program in 2015 is not just
good for your health; it’s also good for your wallet; if you achieve
certain participation targets in 2015, you’ll save on your premiums for
2016; watch for more details coming in early 2015 *
X We are consolidating our FSA program across Nortek with Nortek Offers Four Levels Of
Coverage
UnitedHealthcare; in 2015, all FSAs will be administered through X Employee only
UHC and will automatically be integrated with your medical plan Employee plus spouse or same-
information (if applicable) through myUHC.com; we are also adding X
a new $500 rollover provision for our FSA program; more details sex domestic partner
