LIFE AND ACCIDENTAL DEATH
& DISMEMBERMENT (AD&D)
INSURANCE
It’s important to give some serious thought to what expenses and income needs your dependents would
have if something happened to you. To make sure you have financial protection, HIMSS offers several
different types of Life and AD&D insurance.
BASIC LIFE INSURANCE
HIMSS provides basic Life and AD&D insurance in the amount of one times your annual salary ($50,000
minimum/$350,000 maximum) at no cost. Coverage is automatic.
VOLUNTARY LIFE INSURANCE
You can also purchase additional Life and AD&D insurance for yourself and your eligible dependents. You
may be required to provide proof of good health – also referred to as Evidence of Insurability (EOI).
COVERAGE FOR COVERAGE AVAILABLE
Employee Increments of $10,000 up to 5 times your salary to a maximum of $500,000.
Spouse Increments of $5,000 up to $250,000 - not to exceed 100% of employee voluntary
coverage.
Child(ren) Increments of $2,000 to a maximum of $10,000.
