ELIGIBILITY
If you are a regular full-time employee who works at least 30 hours per week, you are eligible for benefits.
Most of your benefits are effective on the first day of the month following your date of hire. You may also
enroll your eligible dependents for coverage. This includes the following:
Your legal spouse or domestic partner
Children under the age of 26, regardless of student, dependency or marital status
WHEN COVERAGE ENDS
Coverage in the medical, dental and vision plans ends at 11:59 p.m. on the last day of the month in which
your employment ends. All other plan coverages end at 11:59 p.m. on your last day of employment except as
otherwise indicated in this summary.
QUALIFIED LIFE EVENTS
Generally, you may only change your benefit elections during the Open Enrollment period. However,
since life happens, you also may change your benefit elections during the year if you experience a Qualified
Life Event such as:
Marriage, divorce or legal separation
Birth or adoption of a child
Loss or gain of other coverage by the employee or dependent
Eligibility for Medicare or Medicaid
CHANGING BENEFITS AFTER ENROLLMENT
You have 30 days from the Qualified Life Event to make changes to your coverage. Depending on
the type of event, you may need to provide proof of the event, such as a marriage license. If you do
not make the changes within 30 days of the qualified event, you will have to wait until the next Open
Enrollment period to make changes (unless you experience another Qualified Life Event).
