Welcome to 2017 Open It is time for the annual beneits enrollment for the 2017 plan year. Each

beneit needs and adjust your coverage for the upcoming plan year.
Medical .............3 Remember, once you make your selections, you may not change your

Employee Assistance Our open enrollment period will begin on Monday, December 5,
Program (EAP) .......15 2016 and end on Friday, December 23, 2016. Just a reminder, the open
enrollment period allows you to do the following.
Enroll in or waive coverages
Add or remove dependents
Make medical lexible spending account, dependent care lexible
spending account (FSA), or health savings account (HSA) elections
