will be available to answer any questions you may have regarding your coverage
options. Employees will be able to request a copy of the presentation from
Tech Support at 314.621.5884.
We will continue to use Enterprise Web Beneits, through Paylocity, for
enrollment and beneit changes. If you are not making any changes to your
current elections, no action is required. However, there is an exception to the
medical FSA and dependent care FSA. If you currently participate in one of
these accounts, you must re-elect the amount you wish to defer on a pre-tax
basis for 2017. If you would like to switch medical plans, enroll, or waive a
current coverage, you must make those changes in Enterprise Web Beneits.
Medical
We will be remaining with Anthem Blue Cross Blue Shield for the 2017 plan
year. You will continue to have the option between two plans, the PPO and
the HSA. Both plans are considered qualifying coverage under the
Affordable Care Act. Please see the beneits chart found on Page 7 for an
overview of your beneit options.
Each plan has in-network and out-of-network access. When utilizing an
in-network physician or provider, you will receive better discounts and
generally pay less money out-of-pocket. If you choose to utilize an out-of-
network physician or provider, you will be responsible for any charges over
what is considered reasonable and customary, in addition to your member
responsibility through the plan. Charges above reasonable and customary do
not apply to your deductible or out-of-pocket maximum. To locate an in-
network provider, go to www.anthem.com and select “Find a Doctor” under
the “Menu” at the top of the website.
