WELLNESS PROGRAM








Dentons’ wellness program, Wellness for LIfe, offers to help you take control of and
improve your health. To assist with this effort, we have partnered with a wellness vendor
to include their Wellness Program resources.

The Wellness for Life program offers many great features, including a Health Insurance
Premium Discount. If you are enrolled in one of the Firm’s medical plans and fulfill both
wellness requirements each year, you will be eligible for a discount of up to $120 per
year on your premium contributions.

A spouse or domestic partner who is also enrolled in the Firm’s medical plan and meets
both wellness requirements each year can earn you up to an additional $120 per year
in discounted premiums. Incentive rewards are issued as a $2.50 per pay credit toward
your 2021 premium deductions for medical plan coverage for you, and an additional
$2.50 per pay credit toward your 2021 premium deductions for your spouse or domestic
partner.

To fulfill the wellness requirements and qualify for a premium discount, you must
complete an online Health Risk Assessment AND a biometric screening each year by
March 31 to receive a full year’s incentive. Biometric screenings can be certified by your
own physicain or at an on-site screening event. We hope to host an on-site event but we
will want to ensure the health and safety of our partners and employees.

Health and Wellness Program also includes:

Online resource center and much more

The Firm takes the privacy of your health information very seriously. Your screening
and Personal Health Profile will be treated confidentially. No specific individual medical
information will be shared with any partner or employee of the Firm or with any
health insurance carrier. We will, however, use an aggregated summary of all biometric
screenings.
