YOUR 2019 BENEFITS GUIDE
At Patton Logistics, we offer our employees a competitive and
comprehensive beneits program to recognize how important you are to
the company. This beneits guide summarizes Patton Logistics’s program
in a quick and easy-to-understand way. Please review your plan documents
for more details.
Changing Your Benefits Mid-Year
Once you make your elections for the 2019 plan year, you will not be able
to make changes to any of your beneit elections until next year’s annual
enrollment in late November, unless you experience a qualifying life event.
Examples of qualifying events include the following.
Change of legal marital status (e.g., marriage, divorce, death of
spouse, legal separation)
Change in number of dependents (e.g., birth, adoption, death of
dependent, ineligibility due to age)
Change in employment or job status
Please note, you must make changes to your beneits within 30 days of
your qualifying event. If you do not make changes during this time, you
may have to wait until next year’s annual enrollment to make your change.
All elections made during this enrollment period will remain in effect
through December 31. You may only change beneit elections if you
experience a qualifying life event. You may elect or change your Health
Savings Account election at any time throughout the year.
