Consultation and Follow-Up

X Telephone consultation: the physician will call the patient on the number that is
provided generally within an hour and guaranteed within 2 hours

X Video consultation: the physician will schedule a convenient time (guaranteed
same day) to conduct the video consultation—note: you must have a web camera
and high speed internet access to participate in a video consultation
X The physician conducts consultations, recommends treatment, or refers the
patient to a PCP or specialist

X When appropriate, the physician will electronically prescribe medication directly
to the patient’s pharmacy






No beneits will be paid for loss caused by or resulting from: No beneits will be paid for:
X Intentionally self-inlicted injuries, suicide or any attempt while sane or X Eye examinations for glasses; any kind of eye glasses, or vision
insane; prescriptions;
X Declared or undeclared war; X Hearing examinations or hearing aids;
X Serving on full-time active duty in the armed forces; X Dental care or treatment other than care of sound, natural teeth and
