FSA Continued 2016–17 Beneits Enrollment
Submitting a Claim
Our FSA program is administered through UnitedHealthcare. All eligible expenses
must be incurred by December 31 and must be submitted to UnitedHealthcare for
reimbursement by March 31, 2017. FSA claim forms are available at www.myuhc.com.
Employees who enroll will receive 2 MasterCard debit cards. Be sure to keep your
cards at the end of the plan year as old cards will be loaded with new plan year
elections.
Reimbursements from your FSA accounts can be processed when you submit
either a paper form or online submission through www.myuhc.com. Through
www.myuhc.com you can set up convenient direct deposit reimbursements to your
account. If you do not set up direct deposits, reimbursements will be mailed as a
check to you and takes 3–5 days longer to receive your funds.

X Go to www.myuhc.com and log in (or register, if this is your irst visit)
X Click on “Account Settings”
X Select “Manage Direct Deposit”
X Select the accounts you want to enroll in direct deposit and enter your bank
information; have your bank routing and account numbers handy to complete the
short process
Use It or Lose It
Carefully consider your FSA contribution amounts for 2017. At the end of the year
or grace period, you lose any money left over in your FSA. It is important to plan
carefully and not put more money in your FSA than you think you will spend within
a year on things like copayments, coinsurance, prescription drugs, and other allowed
healthcare costs.
FSA Grace Period Extension
ATS’ plan includes a grace period extension, which applies to both our health FSA
and a dependent care FSA. During this time, you may incur eligible FSA expenses
and use the funds remaining in your account from the previous year to cover those
expenses. Participants have from January 1 through March 15, 2018 to incur expenses
and use the remaining funds left in your FSA. Any amounts not incurred during this
time will be forfeited. Employees may ile both health FSA and dependent care FSA 11
claims for expenses incurred during the plan year through March 31, 2018.
