2016 Wellness Program


As a reminder, all wellness program points are tracked through the CHC

Wellness website. All employees are eligible to earn wellness points by
participating in different activities throughout the year. All points will
be tracked through December 20, 2016. See the chart below for a
description of the incentive structure.


The annual premium differential for the 2017 plan year will be $300.
