Life and Accidental Death & Dismemberment Insurance
Life insurance and Accidental Death and Dismemberment (AD&D) coverage by
Voya offers inancial protection for you and your loved ones. The Polyclinic provides
a basic level of coverage at no cost to you. You also have the option to increase the
level of coverage at low cost group rates.
Basic Term Life insurance/AD&D Coverage
X Beneit amount is equal to one times (1x) your annual salary up to $450,000
X Paid for by The Polyclinic
Supplemental Life and AD&D Coverage
X Offered without medical questions to new employees only; if you
choose to elect coverage at a later date, you will have to answer
medical questions and may be denied coverage
X Plan is portable; you can take the coverage with you if you are no
longer employed by The Polyclinic
Employee coverage amount Up to $500,000 ($150,000 guaranteed issue
amount)
50% of employee amount, up to $250,000
Spouse coverage amount ($30,000 guaranteed issue amount)
Child(ren) coverage amount Up to $10,000 guaranteed issue amount
Employee must elect coverage for themselves for spouse and
child(ren) to be eligible.
