Long Term Disability




Long Term Disability (LTD)

Long Term Disability (LTD) by Unum protects your inances by providing you with
income when you are unable to work. There are two different plans to choose from;
one that pays a beneit for two years and a second option that pays a beneit until age
65. Beneits are paid at 60 percent of your salary, up to $5,000 monthly and have a
90-day elimination period.

Own occupation period Option 1: regular occupation for two
years
Option 2: regular occupation to the end
of the beneit duration
Rehabilitation/return-to-work Voluntary program that can provide
assistance program additional beneits and services and
support for employees trying to return
to work; you are eligible to receive
10% of your gross disability payment to
a maximum beneit of $1,000 per week
Dependent care beneit Reimburses for dependent care
expenses (child or adult) when a
disabled employee is participating in
the rehabilitation and return-to-work
assistance program
Worksite modiication beneit Reimburses employer for cost of
workplace modiications that allow
motivated employee to return to work
Survivor beneits Three months standard
Mental and nervous disorder self 24 months
reported symptom
Pre-existing condition limitation 3/12—a pre-existing condition means
a sickness or injury for which you
have received treatment within three
months prior to your effective date; any
disability contributed to or caused by a
pre-existing condition within the irst 12
months of your effective date will not
be covered
