2017 Benefits Enrollment
Eligibility
You may enroll in the beneits program if you are a regular full-time
employee who is actively working a minimum of 30 hours per week.
Your eligible dependents include the following.
Legal spouse, who does not have access to his/her own employer-
sponsored health plan
Dependent children up to age 26
Legal wards; children for whom you are a legal guardian, up to age 26
Plan summaries are detailed in the following pages.
Spousal Certification
If you enroll in the UHC medical plan and your spouse has access to
their employer-sponsored health plan they must elect coverage through
their employer’s plan. Spouses with alternative employer-sponsored health
coverage are not eligible to participate in either UHC medical plan. You
must recertify whether your spouse is currently employed or not. If they
are currently employed and have access to their own employer-sponsored
health plan, you will need to remove them from the UHC medical plan.
