Reliance Standard Plans
AFV will continue to offer 3 plan options administered by Reliance
Standard. These plans are designed to provide you with affordable
options which provide a range of beneits and help satisfy the Individual
Mandate required under the Affordable Care Act (ACA).
The Reliance Standard plans are designed to provide multiple plan
options which allow individuals to choose the beneits to best meet their
speciic needs and budget.
All 3 plan options include the following.
Preventive care
Indemnity beneits
Access to the MultiPlan, Inc. provider network
Prescription drug coverage through Express Scripts
Vision discount card through VSP
24-hr nurse helpline access
Online personal health improvement and wellness tools
OnCall travel assistance
Discounts on vitamins and nutritional supplements
The following pages provide a brief description of each of the services
included in the Reliance Standard plans.
Essential Plan (Preventive Care)
The Essential plan is included in all 3 plan options from Reliance
Standard. It provides you and your enrolled dependents coverage for
preventive care services at no cost to you (meaning you will not have to
pay a copayment, coinsurance, or meet your deductible to receive certain
preventive services).
