ENROLLMENT AND ELIGIBILITY Benefit Eligibility
New Hire Enrollment Status Change

annual enrollment, you can add, change, or decline coverage. In addition,
you can add and/or drop dependents during this time.
Mid-Year Enrollment Changes
Once you make your elections, you will not be able to make changes until
the next annual enrollment period unless you experience a qualifying life
event. A qualifying life event is a change in your personal life which may
impact your eligibility or dependent’s eligibility for beneits. Examples of
some qualifying events include the following.
Change of legal marital status (e.g., marriage, divorce, death of
spouse, legal separation)
Change in number of dependents (e.g., birth, adoption, death of
dependent, ineligibility due to age)
Change in employment or job status
You have 30 days from your qualifying event date to make your change.
If you do not make changes within 30 days, you will need to wait until the
next annual enrollment opportunity. Contact Michelle Geeve in Human
Resources to make changes.
