NextCare offers employees and their eligible family members the
following medical services for a per visit fee of $49.

1. The fee covers the ofice visit including DOT and employment
exams and any in-house labs, x-rays, or other service provided onsite
at the NextCare clinic; should the employee or eligible family member
require outside services (lab, radiology, DME, etc.), the cost of those
services will be the employee’s responsibility and eligible expenses can
be submitted to the employee’s or patient’s insurance plan
The medical services provided through the Employee Visit Program are available at every
NextCare clinic during normal hours of operation . The employee may not be seen when clocked
in as working . The employee must be checked in to be seen as a patient to receive care under
the visit program . The Employee Visit fee of $49 must be paid at time of visit . Employees and
their eligible family members may take advantage of this rate for up to a maximum of 10 visits
per calendar year per family .
