A Closer Look at Eligibility
To be eligible for benefits, you must be an active regular employee
(full time or part-time) working a minimum of 21 hours per week.
You can select one of the four coverage options:
Coverage Option Who Does This Cover?
Employee Only You
Employee + Spouse/DP You and your Spouse or Domestic Partner*
Employee + Child(ren) You and your child(ren) up to age 26
You and:
Employee + Family • Your Spouse or Domestic Partner*
• Child(ren) up to age 26
*There are financial and tax implications you should consider before enrolling a domestic
partner and his or her dependent children for coverage. Contact the BenefitsPlus Solutions
Center at 855.326.7870 for more information.
Making Changes During the 2022 Plan Year
Your benefit elections will remain in effect for the entire plan year
unless you have a qualifying life event, such as a birth, adoption,
marriage, divorce or loss of coverage under another plan. You must
register your changes by visiting www.dentsubenefitsplus.com or by
calling the BenefitsPlus Solutions Center at 855.326.7870 within 31
days of the qualifying event.
