HIGHLIGHTS SHEET
Benefits will become effective on the 1st day of the calendar month following completion of a
30-day waiting period
Must enroll for benefits within first 30 days of eligibility
All regularly scheduled full-time or part-time employees working at least 30 hours per week are
eligible for benefits
The elections made during New Hire or annual Open Enrollment will remain in effect for
the entire plan year (Jan 1st through Dec 31st). Employees may only change coverage if they
experience a qualified “life event” and must do so within 30 days of the event.
Medical Insurance—BlueCross Dental Insurance—
BlueShield of TN Delta Dental of Tennessee
Buy-Up PPO Plan—lower deductible, copay, Value Plan—access to only the Delta
and out-of-pocket maximum, semi-monthly Dental PPO network
premium employee pays will be at a higher rate Base Plan—access to both the Delta
Base PPO Plan—higher deductible, copay, Dental PPO and Delta Dental Premier
and out of pocket maximum, semi-monthly networks
premium employee pays will be at a lower rate
High Deductible Health Plan (HDHP)—higher Vision Insurance—National
deductible, lower premium, employee pays Vision Administrators (NVA)
100% of the cost of prescriptions, doctor visits,
emergency room visits, as well as the cost of Offered to employee and eligible
surgeries and outpatient procedures until dependents

BCBST Programs/Services Health Savings Account (HSA)
Prescription drug PhysicianNow Paired with HDHP medical plan to help
plan telehealth offset cost of medical expenses
Tobacco cessation Nurseline and Allows employee to put pre-tax money
program Nurse Chat aside to help pay for out-of-pocket medical
Maintenance Chronic care expenses (reduces taxable income)
Matters program management Employees enrolled in Medicare are not
Healthy Maternity Diabetes eligible to contribute to an HSA
program… and $3,600 single coverage
