Your Health Risk Assessment
Your Health Risk Assessment
The annual Health Risk Assessment (HRA) is a critical component of our plan
and we strongly encourage your participation. Mitchell Gold + Bob Williams has
partnered with HealthCheck 360 to provide onsite services for the completion of
your annual Health Risk Assessment in mid-November.
Your Health Risk Assessment Will Include
X Health and wellness questionnaire
X Blood pressure
X Height/weight
X Blood test (cholesterol, triglycerides, and blood sugar)
X PSA for men age 40+
All employees and spouses covered under our health plan will need to
complete the HRA to pay the lowest premium.
Upon completion of the annual event, you and your spouse will receive
a healthy living proile indicating your score. Compliance will be based
on a 5% improvement over the prior year’s score. If no improvement
was needed, you and your spouse will be considered compliant. If
you and/or your spouse or domestic partner did not achieve a 5%
improvement over last year’s score, you will be charged an additional
$30 bi-weekly for an employee and an additional $20 bi-weekly for
a spouse or domestic partner. If your 2015/16 healthy living proile
relects the need for improvement, you and your spouse will want to
obtain at least a 5% improvement over your 2015/16 score to pay lower
premiums during the 2016/2017 plan year. Additional details will be
provided in early 2016.
