Lanter offers voluntary plans to help you pay for unexpected costs
associated with an accident or illness. You may purchase voluntary
accident and critical illness coverage for yourself and your dependents.
Voluntary Accident Insurance
This coverage is designed to supplement your medical coverage and is
provided by UnitedHealthcare (UHC). It pays scheduled beneit amounts
to help meet the out-of-pocket expenses and extra bills which can follow
an accidental injury, whether minor or catastrophic, which occurs off the
job. Lump sum beneits are paid tax-free directly to you based on the care
you receive. The wellness beneit provides a beneit of $100 per calendar
year per employee and insured spouse for certain health screening tests.

Employee and Spouse $5.80
Employee and Child(ren) $4.86
Family $6.99
Voluntary Critical Illness
You have the option to elect critical illness coverage for a lump-sum
beneit payment upon diagnosis of a covered illness such as a heart attack,
stroke, or cancer. Coverage is provided by UnitedHealthcare (UHC).
Beneit payments may be used for any purpose you choose and to help
pay the non-medical costs associated with catastrophic illness, such as lost
wages, rehabilitation, family care, or transportation. The critical illness
plan offers a health screening beneit. You and your covered spouse are
eligible for this beneit, which pays $50 per calendar year for any one
health screening test deined in the Plan.
Examples include PSA test, chest x-ray, colonoscopy, pap smear,
mammogram, and more.
Critical illness rates will depend on your age, tobacco status, and the
coverage amount you elect. Spouse rates will be based on the employee’s
age and tobacco status. Rates for this coverage will be provided
during the enrollment process or contact Human Resources.
