Basic Term Life and Accidental Death and
Dismemberment (AD&D) Benefits
The company provides you with a basic term life/AD&D beneit of
2 1/2 times your annual earnings to a maximum of $100,000. Please
note that age reductions do apply. Please update your beneiciaries if
necessary.
Long Term Disability Benefits
Capsa provides all beneits eligible employees with long term disability
coverage. Beneits may be payable after you are determined to be
disabled for more than 90 days. The policy replaces 60 percent of your
income to a maximum of $10,000 per month. Beneits may continue
to the later of Social Security normal retirement age or the policy’s
maximum duration schedule (durations may vary depending on type
of disability).
