Claims Example
Jack is young and relatively healthy but isn’t sure which GLG plan option might be the best bet, considering
past medical and pharmacy expenses. Jack will enroll in single coverage and pay contributions based on a $70K
annual salary. Jack will receive one preventive annual exam, which is covered at 100% on all GLG plans. Jack also
has migraines on occasion and in order to manage them, sees a specialist twice a year. Jack also suffers an ankle
sprain playing basketball at the gym and has to go to urgent care and is prescribed a generic medication to help
ease pain. Jack selects the HSA 1400 plan based on the low payroll contributions and GLG’s annual HSA funding.
