W International provides eligible employees Voluntary Short-Term Disability, Voluntary Long-Term Disability, Group Life
and AD&D, Voluntary Life and AD&D, Voluntary Accident, Voluntary Critical Illness and Voluntary Hospital Insurance
through Unum. In the event you become disabled from a non-work-related injury or sickness, disability income
coverage is a valuable benefit which replaces a portion of your income when you are unable to work.
Life and AD&D insurance provides a benefit to your beneficiary in the event of your death while you are
employed. The AD&D amount is equal to your life insurance amount and is also payable to your beneficiary in the event
your death is a result of an accident. The AD&D insurance may also pay a benefit to you if you have certain injuries.

Weekly Benefit: 60% of your before-tax weekly earnings, not to exceed
$1,000 per week.
Short-Term Disability Elimination Period: Benefits begin on the 8 day of your disabling injury or the 8 day

100% Employee Paid of your disabling illness (7 day elimination period).
Maximum Duration: up to 12 weeks
Benefit: 60% of your before-tax monthly earnings, not to exceed $5,000 per month.
Elimination Period: 90 Calendar Days after the onset of your disabling injury or
Long-Term Disability illness or the date your short-term disability ends.
100% Employee Paid
Maximum Duration: If you become disabled prior to age 60, benefits are payable to age
65, your Social Security Normal Retirement Age, but not less than 5 years. At age 60 (and
older), the benefit period will be based on a reduced duration schedule.
Coverage Amount: $10,000
Group Life and AD&D Benefit Reduction: 65% when you reach age 70 and to 50% when you reach age
100% Employer Paid 75.
Portability is available.
