(4) You enroll in a Medicare Part D plan while the credit union provides prescription drug
coverage,
(5) You do not apply for Part A and Part B of Medicare when you are first eligible,
(6) You die
Your Death after Retirement
If you die after retirement, and your spouse survives you, your surviving spouse continues to
qualify for post-retirement coverage after your death if your spouse qualified for post-retirement
coverage at the time of your retirement and your spouse is covered under the plan on the date
of your death.
Termination of Spouse Coverage
Your spouse’s post-retirement coverage, if any, will terminate on the earliest of the following
events (subject to COBRA coverage continuation rights if the event is a qualifying event):
(1) termination of your coverage for any reason other than your death;
(2) termination of your spouse’s coverage by you or your spouse;
(3) divorce, annulment, or other termination of your marriage; or
(4) death of your spouse.
Death before Your Retirement
If you die before retirement, your spouse (if any) does not qualify for post-retirement coverage,
even if you and your spouse would have qualified for post-retirement coverage had you retired
on the date of your death. Your spouse (if any) may have COBRA coverage continuation rights if
your death is a qualifying event.
Coverage after Normal Retirement Age for Medicare Eligibility
The post-retirement coverage, if any, after you have attained the normal retirement age for
Medicare will be limited to Medicare supplemental coverage and the credit union’s prescription
drug coverage, and will terminate as of the last day of the last monthly coverage period
beginning before you have attained the normal age for Medicare entitlement unless you enroll in
Medicare (including Part A and Part B) and become entitled to Medicare benefits. You must
provide the credit union with a copy of your Medicare card to its effective date as proof that you
have enroll in Medicare and to provide to the health insurance carrier. This also applies to your
spouse after your spouse has attained the normal age for Medicare.
Relation to COBRA Continuation Coverage
This post-retirement coverage is an alternative to, and not in addition to, the COBRA
continuation coverage that may otherwise be available to you and your qualified beneficiaries.
This does not limit the right of you or any of your qualified beneficiaries to elect COBRA
continuation coverage; but this post-retirement coverage is not available unless you and all of
your qualified beneficiaries waive all rights to elect COBRA continuation coverage with respect
to your retirement. COBRA Continuation coverage is not available to any individual eligible for
Medicare benefits.
