Member Driven Technologies (MDT) 2018
Basic Life/Accidental Death & Dismemberment (AD&D)/Optional Life Coverage
Life insurance is extremely important if you have family members that
depend upon your income. Life insurance provides financial security
for you and your dependents should you die while an employee at
MDT. Accidental Death & Dismemberment (AD&D) insurance pays an
additional benefit if your death is a result of an accident. In addition,
AD&D insurance will pay a portion of the benefit for a loss of limb,
eyesight, or both, if the loss is directly related to the accident.
MDT provides a company-paid Basic Life/AD&D benefit and offers
voluntary life benefit for you by Lincoln Financial Group (Lincoln).
Your Basic Life insurance benefits reduce to 65% at age 65, to 40% at
age 70 and to 25% at age 75. Coverage effective dates and increases
in coverage may be delayed if you and/or your dependents are disabled or hospital confined on the date
coverage is scheduled to take effect. Review the Lincoln Summary Plan Description for all details.

Short Term Disability (STD) Coverage
To help protect your financial stability in the event you became ill
or disabled, MDT provides a salary continuation plan through a
short-term disability program also through Lincoln.
Short Term Disability (STD) provides income protection in the
event you become temporarily disabled due to a non-work
related illness or injury. (If you remain disabled when your STD
benefits end, you may be eligible for LTD benefits which are
outline on the following page.)
Your STD benefits are outlines as follows:
